How to Request Medical Records
- Visit our office in person during normal business hours.
- Bring a valid photo ID.
- Let the front desk know you are requesting a copy of your medical records.
- Complete any required records request or authorization form.
- Our team will process the request according to applicable state and federal law.
Please note: We currently only offer medical records requests in person at our physical office.
How to Contact the Applicable Licensing or Disciplinary Authority (Chiropractic Care)
- Identify the provider or clinic involved in your concern.
- Visit the Texas Board of Chiropractic Examiners website.
- Navigate to the “Complaints” or “Enforcement” section.
- Locate the provider using the license lookup tool if needed.
- Follow the instructions on the website to submit a question or complaint.
- Save any confirmation or reference number for your records.
How to File a Consumer Complaint
- Visit the Texas Attorney General Consumer Protection website.
- Select the option to file a consumer complaint.
- Complete the complaint form with your name, contact information, and details about your concern.
- Attach any supporting documents, if applicable.
- Submit the complaint and save a copy of the confirmation for your records.
