How to Request Medical Records

  1. Visit our office in person during normal business hours.
  2. Bring a valid photo ID.
  3. Let the front desk know you are requesting a copy of your medical records.
  4. Complete any required records request or authorization form.
  5. Our team will process the request according to applicable state and federal law.

Please note: We currently only offer medical records requests in person at our physical office.

How to Contact the Applicable Licensing or Disciplinary Authority (Chiropractic Care)

  1. Identify the provider or clinic involved in your concern.
  2. Visit the Texas Board of Chiropractic Examiners website.
  3. Navigate to the “Complaints” or “Enforcement” section.
  4. Locate the provider using the license lookup tool if needed.
  5. Follow the instructions on the website to submit a question or complaint.
  6. Save any confirmation or reference number for your records.

How to File a Consumer Complaint

  1. Visit the Texas Attorney General Consumer Protection website.
  2. Select the option to file a consumer complaint.
  3. Complete the complaint form with your name, contact information, and details about your concern.
  4. Attach any supporting documents, if applicable.
  5. Submit the complaint and save a copy of the confirmation for your records.
